Baptist Health Care, Pensacola Florida. Industrial Hygienist charged with the responsibility of providing an indoor air quality damage assessment resulting from Hurricane Ivan’s impact on sixty buildings in the hospital system during the September 2004 storm. Occupational Environmental Health Solutions, Inc. (OEHS) was retained to conduct an Indoor Environmental Quality Investigation for Baptist Health Care (BHC), headquartered in Pensacola, Florida. Aspects of the investigation included inspection of 752 patient areas located in the facilities. In conjunction with the hospital Risk Management personnel the existing Infection Control Risk Assessment (ICRA) was utilized to develop an Industrial Hygiene Fungal Investigation Strategy which included interstitial moisture readings, collection of interstitial cavity and area non-culturable fungal spore air samples and culturable swab samples from locations within the facilities to identify areas of contamination. Based on the sampling and analytical data, fungal abatement protocols were developed for 573 patient rooms which were identified as having fungal contamination. The ICRA was also utilized to develop the Post Abatement Air Monitoring Strategy for compliance with the Florida Agency for Health Care Administration (AHCA) indoor air quality mandate. The post abatement air monitoring strategy utilized subsequent to fungal abatement activities incorporated a tiered approach which included non-culturable fungal spore and culturable fungal bacteria post abatement air monitoring based on the room occupancy type. The purpose of the investigation was to evaluate indoor air quality within the buildings and determine if there was potential adverse indoor environmental quality or if microorganisms exist in elevated concentrations.

Tri-City Community School District, Fungal Identification & Remediation Design/Oversight, Buffalo, Illinois Conducted the initial investigation, identified the problem, developed the abatement specifications, and provided remediation oversight and post abatement verification monitoring for a 20,000 square foot building addition.

Medical Center Clinic Indoor Air Quality Environmental Survey Pensacola, Florida Conducted an indoor air quality survey of the eleven story 210,000 square foot medical clinic for the purpose of real estate transaction. The survey consisted of the IAQ parameters temperature, relative humidity, dew point, carbon monoxide, carbon dioxide and the monitoring for fungal contamination through the use of culturable bioaerosol air sampling and culturable swab sampling. In addition, a ventilation survey and moisture survey were conducted throughout the facility.

Ambulatory Surgery Center Indoor Air Quality Survey Pensacola, FloridaDesigned the investigation strategy and conducted an indoor air quality investigation the two story 100,000 square foot medical facility which was predicated by Hurricane Ivan. The investigation included collecting non-culturable interstitial cavity, area air sampling and culturable swab sampling in the affected areas of the building. Based on the sampling and analytical data a fungal abatement protocol was developed for implementation by a remediation contractor.

Greater Union Baptist Church Pensacola, Florida Designed and conducted an indoor air quality investigation for the 26,000 square foot church which experienced water intrusion issues as a result of Hurricane Ivan. The sampling and analytical data was utilized to develop a fungal abatement protocol for implementation by a remediation firm.

Beggs and Lane Law Offices Pensacola, Florida Designed and conducted an indoor air quality investigation of the two story 30,000 square foot building which experienced water intrusion issues as a result of Hurricane Ivan. In addition to the law firm A.G. Edwards also occupied about one half of the ground floor of the building. The sampling and analytical data was utilized to develop a fungal abatement protocol for implementation by a remediation firm.

Mt. Zion Intermediate School, Fungal Identification & Remediation Design/Oversight, Mt. Zion, Illinois Identified source of the fungal problem, developed the remediation strategy, provided remediation oversight, and provided clearance air monitoring for a 200,000 square foot school facility which experienced water intrusion issues.

Union Planters Bank Building, Indoor Air Quality Investigation, Shelbyville, Illinois Conducted an investigation of the 100 year old facility which resulted in the identification of fungal contamination.

CHI Overhead Door Company, Bacteria/Fungal Investigation, Arthur, IllinoisConducted an investigation of employee dermatitis complaints. Investigation revealed the presence of bacterial organisms in personal protective equipment.

Wal-Mart Corporation Fungal Identification and Remediation Design/Oversight Numerous locations throughout the United States. Illinois, Indiana, Missouri, North Dakota, New York, Florida. Identifies source of fungal and or bacterial problems, developed the remediation strategy, provided remediation oversight, and conducted clearance air monitoring.

Ball-Chatham Community School District Number 5 Fungal Identification and Remediation/Design, Chatham, Illinois Designed the sampling strategy, identified the fungal contamination issues and developed the remediation strategy through the development of abatement specification for the wings of three separate schools located within the district.

Famous Dave’s Restaurant, Fungal Identification and Remediation Oversight, East Peoria, Illinois Designed the initial investigation sampling strategy for the fungal evaluation of the 25,000 square foot restaurant. In addition, designed and developed the fungal abatement specifications provide abatement oversight and conducted post abatement verification.

Country Place Condominiums, Fungal Contamination Identification and Abatement, Springfield, Illinois Conducted a survey of three units which identified fungal contamination designed and developed the abatement strategy and specifications, provide oversight and air monitoring during abatement activities. Provided final clearance air monitoring services.

The Hope School, Fungal Contamination, Identification, and Abatement Oversight, Springfield Illinois Conducted a survey of the 500,000 ft2 facility for potential fungal contamination. Devised an abatement strategy and provided the oversight and air monitoring during remediation activity.

Illinois State Police Forensic Laboratory Indoor Air Quality Investigation Springfield, Illinois Preformed an indoor air quality investigation of the 30,000 ft2facility which included the identification of fungal contamination and local exhaust ventilation issues.

Cerro Gordo Elementary and Middle Schools Indoor Air Quality Investigation Cerro Gordo, Illinois Conducted an indoor air quality investigation of the two schools. The investigation included monitoring for fungal contamination, moisture evaluations, IAQ parameters, and HVAC review.

Christian County Probation Department Indoor Air Quality Investigation Taylorville, Illinois Developed and performed an investigation of the 20,000 ft2facility. Identified the source of contamination and provided abatement recommendations.

Illinois Emergency Management Agency Indoor Air Quality Investigation Springfield, Illinois. Recently remodeled space was flooded with sewage. The investigation included identifying the areas of drywall that contained greater than 20 percent moisture, conducting bioaerosol monitoring and conducting oversight during the microbiological remediation.

Bank of Springfield, Indoor Air Quality Investigation Commercial Rental Property, Springfield, Illinois Conduced an indoor air quality investigation at a commercial property leased by the State of Illinois. Identified issues were lack of fresh outside air.

Christie Clinic, Indoor Air Quality Investigation, Champaign, IllinoisConducted two investigations at the Christie Clinic. One investigation involved suspect fungal contamination in the Ophthalmology Clinic of the facility. Concern was centered on a moisture intrusion issue due to heavy spring rains. Viable fungal air sampling identified no IAQ related issues. The second investigation was conducted in the Cashiers Office where employees were experiencing a variety of symptoms. Investigation revealed a diverse number of concerns including Inadequate lighting, ergonomic issues, lack of fresh outside air, and job stress.

Gibson City Elementary School, Legionella Sampling, Gibson City, IllinoisDevised and conducted a survey of the potable water supply of the school which identified Legionella bacteria contamination. Provide remediation recommendations and conducted post abatement verification monitoring to document that the contamination was abated.

Rochester High School, Indoor Air Quality Investigation, Rochester, IllinoisConducted an indoor air quality investigation of the high school 1959 wing. The project included fungal spore monitoring, IAQ parameter monitoring and viable fungal swab sampling. The source of the concern was identified as a former exhaust fan opening which wall allowing combustion gases from the boiler to enter a classroom precipitating an odor concern.

Pana Community School District, Fungal Contamination Identification & Remediation, Pana, Illinois Developed sampling strategy and conducted monitoring that identified fungal contamination in areas of the high school and an elementary school in the district. Developed and provided oversight for the abatement project and conducted final clearance air monitoring.

Bunn Capitol Indoor Air Quality Investigation, Springfield, Illinois Designed and conducted annual indoor air quality parameter monitoring at the 300,000 square foot facility. Monitored for temperature, relative humidity, carbon monoxide, and carbon dioxide. Sampling was conducted in order to eliminate union concerns.

Social Security Administration Office Indoor Air Quality Investigations, Green Bay, Wisconsin Industrial Hygienists responsible for investigating indoor air quality complaints based on a computer burn-out. Investigation included ventilation review, microbiological sampling, IAQ parameter sampling, and employee interviews.

Federal Court House Day Care Center, Indoor Air Quality Investigation, Duluth, Minnesota Principal industrial hygienists’ investigator for an indoor air quality investigation, which resulted in significant engineering changes.

University of Florida Citrus Research Station, Indoor Air Quality Investigation, Winter Garden, Florida Industrial Hygienist for conducting an investigation at a research facility that was experiencing Indoor Air Quality concerns because of excessive humidity and lack of adequate fresh air. The cause was determined to be fungal spore infiltration throughout the building, resulting from a lack of conditioned makeup air. The fungal amplification produced spores that cause human immune deficiency problems. A new HVAC system was designed and installed.

Geneseo Schools, Indoor Air Quality Evaluation, Geneseo, Illinois Developed and implemented the inspection and sampling strategy for the entire school district, which comprised eight buildings with approximately 300,000 square feet of space. The inspection and sampling resulted in the identification of several fungal and fresh air health related issues.

University Of Florida Main Library HVAC, Bioaerosol Odor Problem, Gainesville, Florida. Industrial Hygienist for design, investigation and determination of the cause and control measures for a university library that had been experiencing severe odor problems resulting from fungus infestation of the heating, ventilating and air conditioning system.

Illinois Department of Public Health (IDPH) Records Center, Indoor Air Quality Investigation, Springfield, Illinois Conducted an Indoor Air Quality investigation of the IDPH Records Center building. Concern centered on employee symptoms and odor complaints. The investigation identified lack of fresh outside air entering the building and an animal infestation above the drop ceiling.

St. John’s Hospital, Bioaerosol Survey, Springfield, Illinois Designed and conducted a bioaerosol survey of the laboratory section of the hospital. This included collecting samples from suspect material located on the roof. Resolution of the problem was accomplished subsequent to determining that clogged roof drains caused fungal growth, which was entrained by the HVAC system fresh air intake.

State Farm Insurance Company, Residential Fungal Inspections, State of Illinois Conducted residential property evaluations for the presence of fungal contamination. Subsequent to identification of fungal contamination devised the remediation strategies for the affected properties.

Doctor’s Hospital, Indoor Air Quality Evaluation, Springfield, Illinois Principle Industrial Hygiene investigator for an Indoor Air Quality evaluation of an office building located adjacent to the hospital. It was determined that employee symptoms were precipitated by fungal growth under carpeting. The moisture source was determined to be a window air conditioner, which was plumbed improperly.

Southern Illinois University School Of Medicine, Indoor Air Quality Audit, Springfield, Illinois Principal Industrial Hygienist for conducting Indoor Air Quality audit of the prosthesis laboratory. Investigation included review of local exhaust ventilation system effectiveness, HVAC system operation, and Indoor Air Quality parameter monitoring.

State Farm Insurance Company, Bioaerosol Survey, Birmingham, AlabamaPrincipal investigator for the preliminary microbiological sampling to identify the problem areas of the 500,000 square foot facility. Subsequent to problem identification, responsible for the redesign of eight microbiological contaminated HVAC system for facility. This included remedial design, specification development, remediation oversight and air monitoring during abatement activity.

State Of Illinois Building LaSalle Street, Indoor Air Quality Investigation, Chicago, Illinois Conducted bioaerosol air monitoring in the basement areas of the building subsequent to water main rupture and flooding. Subsequent to receiving sample results, remediation recommendations were provided for the elimination of the contamination.

St. Francis Hospital Microbiological, Indoor Air Quality Investigation, Litchfield, Illinois Principal investigator responsible for conducting bioaerosol air monitoring survey at eight locations within the hospital. The survey resulted in the identification of a contaminated HVAC unit in the cafeteria that was subsequently remediated.

Illinois Central Management Service Regional Office Building, Indoor Air Quality Investigation, Springfield, Illinois Principal investigator for conducting an Indoor Air Quality parameter survey to determine the cause of employee health complaints. The survey also included HVAC system effectiveness.

Richland Community College, Indoor Air Quality Survey, Decatur, IllinoisDesigned and conducted Indoor Air Quality parameter monitoring to determine the cause of instructors health complaints. The 300,000 square foot facility was determined to be entraining an exterior source of contamination from a near by commercial facility. The cause was determined to be an HVAC system, which was improperly balanced.

Philips Brother Printing, Indoor Air Quality Investigation, Springfield, Illinois Conducted chemical vapor monitoring and IAQ parameter monitoring to determine the source of employee health complaints.

Illinois Department Of Employment Security, Indoor Air Quality Investigation, Belleville, Illinois Designed and conducted an IAQ parameter survey in the facility to determine the source of employee health complaints.

Veteran Affairs Medical Center Pharmacy, Air Quality Review, Ann Arbor, Michigan Industrial Hygienist for design and review of a government-operated hospital pharmacy, located in southeastern Michigan, where employees were experiencing adverse health effects. The source of the illnesses was traced to a rare case of mass psychogenic illness related to poor Indoor Air Quality. Recommendations helped resolve employee dissatisfaction and dispel the issue.

U.S. Customs Services Department, Indoor Air Quality Audit, Detroit, Michigan Industrial Hygienist for design of an audit for the U.S. Customs Department facility located on an international border crossing. Employees were concerned about elevated carbon monoxide, lead and hydrocarbon levels within the inspection facilities. Recommendations resulted in the construction of new facilities with an improved ventilation system.

U.S. Army Corps Of Engineers Facilities, Indoor Air Quality Audit, Detroit, Michigan Industrial Hygienist for design and management of an audit for the U.S. Corps of Engineers facilities located in Detroit, Michigan. The audit revealed elevated levels of solvent vapor, resulting in the development of improved respiratory protection and confined space entry techniques.

Michigan Social Security Administration, Indoor Air Quality Audit, Various Locations Throughout Michigan Industrial Hygienist for design and management of a survey for all the Social Security Administration offices within the state of Michigan. The purpose of the audit was to determine the potential for inadequate Indoor Air Quality. The audit included monitoring typical IAQ parameters plus radon gas. Recommendations for remedial action were presented for locations where issues of concern were identified.

Franklin Life Insurance Company, Bioaerosol Monitoring, Springfield, Illinois Principal Industrial Hygienist for bioaerosol monitoring and IAQ parameter monitoring on one floor of the seven-floor building. Employee complaints were determined to be the result of fungal and bacterial growth, which was the result of a water intrusion problem.

Lincoln Correctional Center, Indoor Air Quality Investigation, Lincoln, Illinois Designed and conducted, an Indoor Air Quality investigation in the office complex of the correctional facility. Remedial activities included providing greater quantities of fresh air and increasing the relative humidity.

Southern Illinois School of Medicine, Bioaerosol Survey, Springfield, Illinois Designed and conducted a bioaerosol survey of an office facility. Provided recommendations for the abatement and cleaning of the facility HVAC system and contaminated basement area. This included remedial design, specification development, remediation oversight and air monitoring during abatement activity.

Willard Ice Building-Illinois Revenue Building, Bioaerosol Survey, Springfield, Illinois Conducted a bioaerosol/microbiological survey of the 650,000 square foot facility. This included bioaerosol air monitoring, HVAC microbiological bulk/swab sample collection, building components microbiological bulk/swab sample collection, and Legionella water sample collection.

Bank One, Investigation And Recommendation Of Remedial Action, Rockford, Illinois Principal Industrial Hygienist for conducting a complex IAQ investigation and recommendation of remedial action for a large office complex experiencing Indoor Air Quality concerns because of inadequate fresh air supply. The source of the problem was determined to be a very rare mass psychogenic illness.

Residential Indoor Air Quality Investigations, OEHS, Inc. has conducted hundreds of residential IAQ investigations. This includes inspections for fungal and bacterial infestation as well as inadequate ventilation, and inappropriate IAQ parameters such as carbon monoxide, carbon dioxide, temperature, and humidity.

TRADITIONAL INDUSTRIAL HYGIENE

Cray Research, Noise Surveys, Chippewa Falls, Wisconsin Industrial Hygienist for design of numerous comprehensive industrial noise surveys. This involved determining a statistically accurate number of sampling locations, identifying employees that exceeded the Permissible Exposure Level and recommending the type of worker Personal Protective Equipment and/or engineering control measures to provide compliance with OSHA standards.

Illinois Environmental Protection Agency, Ambient Air Monitoring, Various Locations, Illinois Industrial Hygienist for conducting ambient air monitoring to determine compliance with the Clean Air Act during the incineration of debris generated during the 1993 Great Mississippi Flood.

AC&R Components Plasma Arch, Isokinetic Air Monitoring Survey, Chatham, Illinois Designed and conducted Industrial Hygiene air monitoring for the welding and plasma arch equipment in the facility. The consequences of the survey resulted in the instillation of additional local exhaust ventilation.

United States Postal Service, Industrial Hygiene Air Monitoring, Springfield, Illinois Conducted air monitoring at post offices to determine the source of employee health complaints.

Salvaggio Steel, Industrial Hygiene Air Monitoring Survey, Springfield, Illinois Designed and conducted an Industrial Hygiene air monitoring survey of the steel fabrication facility. Findings resulted in the additional local exhaust ventilation to eliminate contaminates in the employees breathing zone.

Pal Health Systems; Pekin, Illinois Industrial hygienists for conducting personal and air integrated air monitoring for the manufacture of arch supports. Toluene, Acetone, MEK, Hexane, Ethyl Acetone, & Cyclohexane.

Rutland Products, Jacksonville, Illinois Principal industrial hygienist for designing and conducting ventilation survey for three mixer/Muller local exhaust ventilation hoods and associated ductwork system. Survey included collecting hood face velocity readings, conducting pitot traverse to determine the duct transport velocities and total volumetric flow rates.

Rutland Products, Jacksonville, Illinois Designed and conducted an air monitoring survey for total and respirable dust. The purpose of the survey was to document adequate ventilation subsequent to redesign of the exhaust system.

Rapps Engineering, Springfield, Illinois, Developed the site health and safety plan for the phase three environmental investigation of a former cabinet manufacturing facility which was destroyed by fire. In addition, provided onsite health and safety oversight during the investigation and conducted air monitoring to document property line chemical concentrations. Also provide respiratory protection training and fit testing for twenty employees who conducted the investigation.

University of Illinois at Springfield, Springfield, Illinois Provide onsite training for employees. Topics included lock out/tag out, confined space entry, asbestos awareness, and asbestos operation & maintenance

Maxwell Counters, Farmer City, Illinois, Conducted total and respirable dust monitoring for the counter top manufacturing facility. Contaminants included both wood and manmade materials. Assisted in the design of a local exhaust ventilation system to eliminate the dust exposure issues.

Rutland Products, Jacksonville, Illinois Principal industrial hygienist for designing and conducting ventilation survey for three mixer/Muller local exhaust ventilation hoods and associated ductwork system. Survey included collecting hood face velocity readings, conducting pitot traverse to determine the duct transport velocities and total volumetric flow rates.

Pioneer Surgical Technology, Gaylord, Michigan, Developed a sampling strategy for the identification of chemical contamination at a former U.S. Air Force facility. Activities included the collection of samples, review of analytical data, and development of a report.

Christie Clinic, Champaign, Illinois, Designed and developed a sampling strategy for the clinic which included light survey, carbon monoxide, carbon dioxide, ergonomics and heat stress. Findings and recommendations assisted in the reduction of employee complaints and lost time.

Veterans Affairs Medical Center, Occupational Health Medical Surveillance Program, Marion, Indiana Industrial Hygienist for management, development and implementation of an occupational health medical surveillance program for a large 1,200-bed hospital located in northern Indiana. This project involved classifying occupations by type of exposure and working with the medical staff to recommend appropriate examination and exam frequency to provide adequate employee protection for the anticipated occupational stresses.

University of Florida Research Station, Winter Garden, Florida Industrial Hygienist for conducting advanced air monitoring techniques to evaluate employee exposures to numerous chemicals used in hospital laboratories.

University of Florida Research Station, Gainesville, Florida Industrial Hygienist for design and implementation of the cleanup of an electron microscope laboratory contaminated with osmium tetraoxide. This involved developing a written action plan, directing the remediation activity and conducting the personal, area and perimeter air monitoring to document the cleanup procedures.

Central Illinois Public Service, Air monitoring, Taylorville, Illinois Industrial Hygienist for the design of the sampling strategy and implementation of air monitoring for a construction project that involved soil contaminated with coal tar pitch volatiles.

Southern Illinois University School of Medicine Prosthesis Laboratory, Industrial Hygiene Survey, Springfield, Illinois Designed and conducted an Industrial Hygiene survey of the prosthesis laboratory to determination carbon fiber size. This resulted in additional local exhaust ventilation and respiratory protection for the employees.

Illinois Department Of Nuclear Safety, Fit Testing; Springfield, IllinoisIndustrial Hygienist for quantitative respiratory fit testing.

General Dynamic, Health And Safety Plans, Warren, Michigan Industrial Hygienist for design and implementation of numerous site health and safety plans for all types of environmental remediation activities. These have included plans for generic tank removals, chrome and chromic acid spills, medical waste cleanups, and soil boring locations.

Great Western Abatement, Jacksonville, Illinois Developed and presented an eight hour refresher course for compliance with the OSHA standard Hazardous Worker Protection.

11th Street Bridge, Lead Air Monitoring, Springfield, Illinois Industrial Hygienist for conducting a lead air monitoring survey for the construction on the bridge.

This involved auditing all the generating stations and office facilities, recommending compliance actions, implementing training requirements and developing and executing the written program manual.

Yard, Chicago, Illinois Industrial Hygienist for conducting a feasibility study as part of a site-specific health and safety plan for a 317-acre facility contaminated with PCB’s. This project also involved the development and implementation plan, which included working within the constraints of an urban setting and in conjunction with several other firms.
